Evaluation and treatment of anxiety including situational (reactive) anxiety, generalized anxiety, panic attacks, social anxiety, social avoidance or ambivalence (social confidence, trust and intimacy issues), and PTSD (See Trauma Psychology below); also distress with current relationships and life circumstance or event or illness-related emotional adjustment and stress coping difficulties, including self-esteem and social confidence issues related to ADHD Evaluation and treatment of depression including situational (reactive) depression, major depressive disorder, persistent depressive disorder, depression with mixed features, depression with brief hypomania, cyclothymic disorder, and bipolar II depressive disorder Consult-liaison with Psychiatry for the conditions above when medication is indicated, concurrent with psychotherapy Evaluation and treatment of post-trauma stress and stress & trauma disorder/ PTSD (*) (negative moods, hypervigilance or easy startle, trauma re-experiencing with nightmares and/or intrusive daytime trauma memories, and emotional numbing or other kinds of trauma avoidance or disassociation-type 'blocking') persisting after experiencing or witnessing a traumatic event (e.g. death or violent or otherwise life-threatening or compromising event); or after learning that such an event has occurred to a person (or persons) with whom there is a close relationship (this kind of PTSD stressor/onset often including traumatic bereavement (i.e. complicated grief related to surviving the violent death and/or wrongful and/or unnatural-unnecessary death of a loved one or loved ones) (*) I do not treat most cases of Complex PTSD, e.g. stress and trauma symptoms due to childhood abuse, neglect or abandonment or ongoing domestic violence or abuse |
Neuropsychological evaluation per Neurology or PCP referral -standardized neurocognitive testing (quantified measurement) of functions to include attention, concentration, memory, language, spatial perception, and neuro-executive skills (e.g. multitasking or divided and selective attention, attentional and conceptual flexibility, planning, organization, step sequencing and completion, and problem-solving); also neurobehavioral examination focusing on decision-making, social judgment, impulse control, mood regulation, and stress coping / stress tolerance) Neuropsychological evaluation and medical consult-liaison to help referring providers with differential diagnosis and prognosis pertaining to established or suspected central nervous system conditions: pseudodementia (question of depression vs. dementia), dementia such as Alzheimer's or vascular (ischemic disease) or frontal lobe dementia (e.g. primary progressive aphasia), stroke (CVA), Parkinson's, Multiple Sclerosis (MS), Hydrocephalus (NPH), and other CNS degenerative diseases and transient organic brain syndromes

CANCELLATION(S): Should you need to or decide to cancel your initial appointment with Dr. Burkhart or any follow-up appointments you have scheduled, the courtesy of a 72-hour notice is appreciated. There will be no charge for appointments canceled 48 hours or more in advance. Late cancellation and no-show fees cannot be billed to your insurance company. Please make every effort to keep your appointment(s). While there are other patients scheduled and on a waiting list to be moved up to fill cancelled appointments, 48 hours is usually required for them to make all the necessary arrangements.
PAYMENT TERMS: If you have signed the New Patient Registration Form (Intake Sheet) and authorized payment of health insurance benefits to Dr. Burkhart, it is agreed that you will ultimately be responsible for any portion of amounts charged, not paid by your insurance company. If Dr. Burkhart is a participating provider with your insurance, he has agreed to an allowed fee amount and after your insurance is billed and paid their portion of that amount, you will be billed for whatever co-insurance and/or co-payment is due from you. Your insurance deductible may be applied and so you may end up being responsible for the full amounts allowed by your insurance company. And should the insurance or eligibility status listed on your Intake Sheet and initially verified by Dr. Burkhart change by the time of your appointment and if by then you have not provided and had verified alternative insurance, you will then be responsible for any and all fee amounts charged. |
Getting to Ballinger Professional Offices
From points south / traveling North on I-5: Take Exit 177 and follow signs toward Lake Forest Park. Pass through the first intersection where you’ll see a Chevron station on your right and head down Ballinger Way. From points north / traveling South on I-5: Take Exit 177 and follow signs toward Lake Forest Park. At the end of the off ramp you’ll turn left and head under the I-5 overpass and then pass through the first intersection where you’ll see a Chevron station on your right and head down Ballinger Way. Headed down Ballinger Way right east of I-5, you’ll drive past Ballinger Village (and the Village Market Thriftway) on your left and then a second Chevron station also on your left at the corner of the next intersection, which is 19th Ave NE. Here, take a left and head up the hill to the middle of the block and you’ll see Dr. Burkhart’s office on the right, a one-story olive-colored building. Parking is usually available in the lot in front of the building. Upon arrival inside the office, please have a seat in the waiting room and you will be greeted there, when it is time for your appointment. |
