How to Make An Appointment
​Patients or healthcare providers with referrals for a neuropsychological evaluation or for behavioral pain or medical rehabilitation and other medical psychology referrals, please start by calling Dr. Burkhart’s office at 206-365-1435 and press 1 to leave a message for Lee Farley, his patient intake coordinator and psychometrist. She will answer any questions you may have and if you wish to proceed, schedule an initial appointment with Dr. Burkhart. She will also explain to you what forms need to be completed and/or signed in preparation for your visit (downloadable fillable forms posted below, which can be downloaded to your computer, filled out on your computer, and then either emailed to the office before your visit or printed and mailed or accompany you to your visit).

If you are interested in other psychological services, e.g. wanting evaluation and consultation for the purposes of psychotherapy with Dr. Burkhart, please note that he is 2+ months out in accepting new patients.  But if you wish to wait and your treatment needs allow it, you can email him at DrBurkhart@waburkhartphd.com and describe the symptoms or problems for which you are needing help, along with any history or other background information which may be relevant to your treatment needs and reasons for seeking help at this time.  Also helpful, let him know what your funding source would be, what health insurance you have or whether or not you would be paying privately. And if your treatment needs and goals are a good match for his areas of specialty and expertise and if he takes your insurance or private pay is not a problem, he will follow-up with you for more information and discussion; and then if you and he agree to do so, an appointment will be scheduled.

Please note that if you experiencing or wanting help with a potentially life threatening emergency (acutely dangerous suicidality or risk of harming others) and not an established patient with Dr. Burkhart, you are advised to call 911, visit your nearest ER or Urgent Care Clinic, or call the King County Crisis Line at 206-461-3222 or the Snohomish County Crisis Line at 425-258-4357.

Owing to the coronavirus crisis, since Mid-March insurance companies have been temporarily allowing 'telehealth' psychotherapy, including evaluation/testing and consultation prior to and in preparation for treatment (https://doxy.me/drburkhart). And this funding is likely to remain in place into the fall and maybe through the end of the year, depending on your insurance company. But as of June 15, 2020, in-person office psychological evaluation and psychotherapy visits have resumed, an option but not a requirement. For now visit by visit, you can decide if you want to complete your visit in the office or via 'telehealth'. 

Also as of June 15, 2020, Dr. Burkhart's in-office neuropsychological evaluation services have resumed.

If you are scheduled for a neuropsychological or psychological evaluation or if you elect to resume in-person office psychotherapy, in preparation please download, review, and sign the COVID19 SAFETY CONSENT FORM form (Download File below). Your consent to comply with the safety precautions specified  in this form is required  before your first in-office visit and between visits. Also per this consent, you will be informed about the specific COVID19 safety cleaning, masking, and distancing protocols to which  Dr. Burkhart and his staff are committed and with which they will be complying prior to and after your office visits(s).

​CANCELLATION(S): Should you need to or decide to cancel your initial appointment with Dr. Burkhart or any follow-up appointments you have scheduled, the courtesy of a 72-hour notice is appreciated.  There will be no charge for appointments canceled 48 hours or more in advance. Late cancellation and no-show fees cannot be billed to your insurance company.  Please make every effort to keep your appointment(s). While there are other patients scheduled and on a waiting list to be moved up to fill cancelled appointments, 48 hours is usually required for them to make all the necessary arrangements.
 
PAYMENT TERMS: If you have signed the New Patient Registration Form (Intake Sheet) and authorized payment of health insurance benefits to Dr. Burkhart, it is agreed that you will ultimately be responsible for any portion of amounts charged, not paid by your insurance company.  If Dr. Burkhart is a participating provider with your insurance, he has agreed to an allowed fee amount and after your insurance is billed and paid their portion of that amount, you will be billed for whatever co-insurance and/or co-payment is due from you. Your insurance deductible may be applied and so you may end up being responsible for the full amounts allowed by your insurance company. And should the insurance or eligibility status listed on your Intake Sheet and initially verified by Dr. Burkhart change by the time of your appointment and if by then you have not provided and had verified alternative insurance, you will then be responsible for any and all fee amounts charged.
Getting to Ballinger Professional Offices

From points south / traveling North on I-5: Take Exit 177 and follow signs toward Lake Forest Park. Pass through the first intersection where you’ll see a Chevron station on your right and head down Ballinger Way.

 
From points north / traveling South on I-5: Take Exit 177 and follow signs toward Lake Forest Park. At the end of the off ramp you’ll turn left and head under the I-5 overpass and then pass through the first intersection where you’ll see a Chevron station on your right and head down Ballinger Way.

Headed down Ballinger Way right east of I-5, you’ll drive past Ballinger Village (and the Village Market Thriftway) on your left and then a second Chevron station also on your left at the corner of the next intersection, which is 19th Ave NE. Here, take a left and head up the hill to the middle of the block and you’ll see Dr. Burkhart’s office on the right, a one-story olive-colored building.

Parking is usually available in the lot in front of the building. Upon arrival inside the office, please have a seat in the waiting room and you will be greeted there, when it is time for your appointment.
